ACMA Seeking Expert in Four Clinical Areas
ACMA is seeking a case management professional with expertise in endocrine-specific conditions - specifically diabetes, thyroid disease, osteoporosis and metabolic syndrome - to nominate to a National Quality Forum (NQF) Expert Standing Committee. The committee will oversee NQF's portfolio of endocrine measures that includes performance measures for accountability and quality improvement that specifically address endocrine conditions. The committee's primary work is to evaluate newly submitted measures and measures undergoing maintenance review against NQF's standard measure evaluation criteria and make recommendations for endorsement. The committee term will be two to three years, and members should be available to participate in regular calls or meetings. Application for the Franklin Award of Distinction
for Transitions of Care Available Online

The 2014 Franklin Award of Distinction for Transitions of Care application is available online. Newly redesigned to account for recent changes in health care, the Franklin Award now extends its focus to recognize exemplary transitions of care in case management.

The Franklin Award is presented annually by ACMA and The Joint Commission to an organization that exhibits excellence and distinction in hospital/health system case management and transitions of care. Pursuing this national award for excellence in hospital case management and transitions of care provides a unique opportunity to promote the advancement and recognition of case management within your organization, as well as an opportunity to scrutinize your model and processes. Both ACMA and The Joint Commission encourage all organizations with distinctive case management departments to apply.

"Since You Asked" Committee Update
 
As introduced in the most recent issue of Collaborative Case Management, the "Since You Asked" Committee is an interdisciplinary group of experienced case management professionals committed to answering questions relevant to the practice. In the future, the committee will provide regular updates to this newsletter.

Community Acquired Pneumonia (CAP)...When is hospitalization appropriate?

Community Acquired Pneumonia (CAP) can be managed in the outpatient setting. There are times when it is appropriate to hospitalize these patients. Case managers are tasked with applying objective clinical criteria such as InterQual or Milliman Care Guidelines to guide medical necessity determinations for appropriateness of the treatment setting and level of care. There are times when the patient's clinical condition indicators fall short in meeting the medical necessity for hospitalization. Cases that do not meet the medical necessity criteria for the setting and/or level of care should be referred to a secondary physician reviewer.

An additional piece of information that a case manager could provide to the secondary physician reviewer is Pneumonia Severity Index (PSI) score and/or the CURB-65 score. These prospectively validated clinical prediction tools, endorsed by The American Thoracic Society (ATS) and Infectious Diseases Society of America (IDSA), assist the physician to determine which setting, outpatient or inpatient, is clinically appropriate for the patient - read more...
