Administration on Aging Website Offers Care Transitions Information and Resources
The Administration on Aging (AoA) website offers a number of resources related to Transitions of Care designed to aid patients and families, as well as administrators and practitioners. The site features a “Preparing for Discharge” section, which includes a discharge planning checklist for patients and caregivers. The AoA site also includes a link to a document titled “Taking Care of Myself: A Guide for When I Leave the Hospital,” which provides a way for patients to track their medication schedules, upcoming medical appointments, and important phone numbers. Free Tools Available to Hospitals Through Project RED
 
Many ACMA members are familiar with the Project RED program, designed to provide hospitals with access to tools meant to lower patient readmission rates. Project Red — for Re-Engineered Discharge — was developed by Boston University and is funded by the Agency for Healthcare Research and Quality (AHRQ). In a study at Boston University Medical Center the program and its after-hospital care plan for patients reduced readmissions by 30%.

In a recent piece on The Wall Street Journal’s Health Blog, Brian Jack, the Boston University researcher who developed Project RED, says that under a new contract with AHRQ his team is developing a toolkit explaining how to implement the program and is testing different implementation strategies at 10 hospitals.
