Healthcare News & Legislative Update Patient Safety Webinar Focuses on Reducing Readmissions Through Care Transitions Today, the National Quality Forum (NQF), the Partnership for Patients and the National Priorities Partnership (NPP) will host a free webinar titled “Reducing Readmissions Through Care Transitions.” The session is the second installment in an eight-part patient safety webinar series.
