Colorado Hospitals Reduce Avoidable Readmissions, Save $2.9 million
According to a recent announcement from the Colorado Hospital Association (CHA), a joint initiative between CHA and UnitedHealthcare to reduce readmissions at 19 Colorado hospitals has shown significant outcomes in its first year of implementation. The results show a decrease in "same cause" readmissions within 30 days of discharge from 9.8% to 5.18% for target populations, while "all cause" readmissions fell from 14.9% to 8.5%. Franklin Award of Distinction for Transitions of Care
2014 Application Now Available

Newly redesigned to account for recent changes in health care, the Franklin Award now extends its focus to recognize exemplary transitions of care in case management.

The Franklin Award is presented annually by ACMA and The Joint Commission to an organization that exhibits excellence and distinction in hospital/health system case management and transitions of care. Pursuing this national award for excellence in hospital case management and transitions of care provides a unique opportunity to promote the advancement and recognition of case management within your organization, as well as an opportunity to scrutinize your model and processes.

Transitions between care settings is not a new concept for case management professionals. Case managers understand that a safe, efficient transition can make all the difference for a patient. However, with increased scrutiny and penalties for excessive readmissions, organizations nationwide are reevaluating existing models and implementing innovative programs to help ensure patients transition safely and remain out of the hospital.

A two-day intensive workshop November 6-7 in Las Vegas, the 2013 ACMA Leadership Conference is focused specifically on Transitions of Care. The event's experienced faculty consists of industry experts and health care leaders who will discuss community partnerships, provide tools, and present examples and successful models that can be replicated and incorporated into your current practice.

The conference is designed for hospital/health system case management professionals in leadership positions. The conference is a two-day intensive workshop focused on transitions of care. Sessions will focus on a variety of issues affecting case management leaders, including:
  • Building new community relationships
  • Addressing health insurance exchanges
  • Helping patients grasp transitions of care
  • Maintaining the integrity and distinction of the role of case managers
