Last Chance to Contact Your Congressman at Home
As the August recess winds to an end, there are only a few days remaining to meet with your Senators or Representative in one of their local offices to request their support of the Improving Access to Medicare Coverage Act of 2013 (H.R. 1179 or S.569) and the Medicare Audit Improvement Act of 2013 (H.R.1250 or S.1012). Invite them to your hospital or health system and let them see firsthand your role in reducing readmissions, quality improvements and care coordination. Be sure to tell them how observation services and the three-day stay requirement are creating barriers for case managers in providing appropriate care and facilitating safe transitions. Explain the burden that recovery audit contractors (RACs) are placing on your organization's resources.

Take advantage of this opportunity to make your voice heard and ensure that Congress is aware of the issues impacting hospital case management practice. Recognize Those Who Make A Difference
The ACMA Fundraising Committee's In Memory/Honor fundraising initiative has returned for 2013. Coinciding with National Case Management Week, this program provides case management professionals an opportunity to donate a gift in honor of, or in memory of, an individual, department, chapter or organization. This is a great opportunity to honor or remember those who have not only impacted you personally in your career in health care, but those who daily make a difference in the lives of patients and all of those they touch.

By recognizing an individual or group, their name will be listed: in ACMA's Collaborative Case Management journal, in ACMA's briefCASE e-newsletter and an email announcement during National Case Management Week, on the ACMA website, at the 2013 ACMA Leadership Conference, and at the 2014 ACMA National Conference.

Register Now for the 2013 Leadership Conference

Transitions between care settings is not a new concept for case management professionals. Case managers understand that a safe, efficient transition can make all the difference for a patient. However, with increased scrutiny and penalties for excessive readmissions, organizations nationwide are reevaluating existing models and implementing innovative programs to help ensure patients transition safely and remain out of the hospital.

A two-day intensive workshop November 6-7 in Las Vegas, the 2013 ACMA Leadership Conference is focused specifically on Transitions of Care. The event's experienced faculty consists of industry experts and health care leaders who will discuss community partnerships, provide tools, and present examples and successful models that can be replicated and incorporated into your current practice.

The conference is designed for hospital/health system case management professionals in leadership positions. The conference is a two-day intensive workshop focused on transitions of care. Sessions will focus on a variety of issues affecting case management leaders, including:
  • Building new community relationships
  • Addressing health insurance exchanges
  • Helping patients grasp transitions of care
  • Maintaining the integrity and distinction of the role of case managers
