Healthcare News & Legislative Update Hospital ACO Model Reduces Readmissions, Saves $400,000 According to a recent article in the Miami Herald, a care coordination pilot program at Jackson Memorial Hospital in Miami, Florida, has shown promising results in improved outcomes. The program aims to reduce readmissions through follow-up care, and since its implementation, has seen readmission rates in heart patients decrease by one third. The program also saved the hospital $400,000 in readmission charges. The event will feature a number of thought leaders and national experts in the health care field. Sessions will focus on a variety of issues impacting case management leaders, including: readmissions, ACOs, bundled payment and care transitions.
