Eastern Pennsylvania Chapter of ACMA (est.2012)

Represents Case Management professionals, Registered Nurses, Social Workers, Physician Advisors, Case Management Assistants, Nurse Practitioners, and Care Transitions staff, who reside or work in Eastern Pennsylvania and/or neighboring states without a local chapter.

The ACMA Eastern Pennsylvania Chapter is accepting presentation proposals for our 2018 ACMA EPA Chapter Annual Conference. We are searching for engaging speakers and practitioners with innovative solutions, which have been tested in daily practice. Submission deadline: Friday, August 25, 2017. Event information below:

  • Successful initiatives or models
  • Effective tools or interventions
  • Programs that have demonstrated successful outcomes
  • Evidence-based research
  • New information on key topics in health care delivery system case management and transitions of care

Are You Someone Or Do You Know Someone Who Fits the Bill?

Largely, the committee is looking for presentations relating to the following topics/areas:

  • Navigating the continuum of care, dealing with the opioid addiction crisis
  • Hospice and advance life care planning, how to intervene early
  • Innovative case management projects throughout the care continuum
        • Convey original solutions, interventions, or strategies
        • Provide tangible tools or models for intervention

      Generally, proposals submitted by healthcare vendors/consultants and those submitted by practitioners for promoting specific vendor products are not eligible and will not be submitted to the committee for review.  ACMA defines a health care vendor as a company, organization, and/or individual that promotes or sells products and services to which the patient is not the ultimate purchaser / decision maker but instead case management professionals and/or their employer are the purchasers. We are planning a great conference for you and want you to be a part of it!  We look forward to showcasing our own local Eastern Pennsylvania Case Management experts!
