We represent Case Management professionals, RNs, Social Workers, Physician Advisors, Case Management Assistants, Nurse Practitioners, and Care Transitions staff who reside and/or work in the state of New York.

Annual Meeting
Join us for the 9th Annual New York Chapter Conference to be held on September 12, 2025 at Hilton Long Island/Huntington in Melville, New York.
The event will be an opportunity to share knowledge and learn with case management and transitions of care professionals in health care. Take advantage of this unique professional development event right in our backyard and improve patient outcomes.

Virtual CE Event with the NY Chapter
