Speaker: Rose M. Turner, RN, BSN, ACM-RN Meeting the needs of every hospital discharged patient can be challenging. We needed to do more for patients who could not easily access healthcare. They waited too long to seek care for their chronic illnesses and arrived in our ED instead of an MD office or clinic. In collaboration with county public health nurses and county fire EMTs, we created a no cost program to visit our patients who could not get standard post-acute services to their home. Unlimited visits and phone calls, basic health care tools and DME are provided as well as teaching. One-year results are so great, we have now been funded to continue and expand to provide a sustainable program.
Course Objectives:
- Identify patients discharged from home with post-acute needs that cannot be met by contracted providers.
- Develop KPIs for a program to measure success for continuation.
- Utilize partners/resources in the community to assist in a pilot program.
