Trump Administration and CMS Announce Rural Health Initiatives

August 11, 2020. Using the Affordable Care Act as the basis for his recent Executive Order, the Trump Administration announced it is setting the stage to transform the rural healthcare system by providing new funding opportunities to increase access and improve quality. The Community Health Access and Rural Transformation (CHART) Model aims to ensure rural America has access to high quality, affordable health care.

This Model offers two tracks for participation. The Community Transformation Track provides for expansion of telehealth to allow a beneficiary’s place of residence to be considered an originating site and it waives certain Conditions of Participation to permit a rural outpatient department and emergency room to be paid as if they were in a hospital. It also allows participating hospitals to waive cost sharing for some Part B services and provide transportation and gift cards for chronic disease management. This track is slated to begin in the summer of 2021.

The second track is the Accountable Care Organization (ACO) Transformation Track offers upfront investment to assist rural healthcare providers in improving outcomes and quality for rural beneficiaries. Providers participating in the ACO Transformation Track will enter into two-sided risk arrangements as part of the Medicare Shared Savings Program (MSSP). It is slated to begin in January 2022.
