Care Transition Rounds: A System-Wide Initiative to Reduce Length of Stay

Abstract
Opportunities for improvement were identified when leaders within this health system identified wide variation across patient care units and hospitals related to duration of the rounds and content discussed for each patient. Development and implementation of standardized discharge planning rounds (Care Transition Rounds-CTRs) across 12 hospitals as a length of stay reduction initiative was implemented. Eleven core elements were identified to be reviewed for each patient daily (M-F) with the discussion time of one minute per patient to enhance communication among the care team for patients' care progression and discharge readiness. Coupled with an escalation hierarchy to address and remove barriers to timely discharge, CTRs have successfully reduced length of stay and excess days.

2. Recognize delay reasons contributing to excess days and an increased length of stay.
3. Design an interactive dashboard to monitor excess day and length of stay data by unit, hospital, facility, region and system.

Speaker: Stacy Galik, LMHC, CCM | Director, Hospital Care Coordination | BayCare Health System, West Region

Stacy Galik is the Director of Hospital Care Coordination for five acute care hospitals within the BayCare Health System, a large, non-profit health care system in the greater Tampa Bay area. Stacy’s departments are responsible for managing hospital length of stay, reducing readmissions, oversight of observation patients, discharge coordination to the appropriate post-acute level of care and contributing to a positive patient experience. For the past 14 years, Stacy has worked in hospital case management/social services in both medical and behavioral health programs. She received her bachelor’s degree in Psychology from the University of South Florida and Master’s Degree in Rehabilitation and Mental Health Counseling also from the University of South Florida. Stacy is an active member of the American Case Management Association (ACMA) and the Morton Plant Mease Ethics Committee. Stacy is a Certified Case Manager (CCM) and serves as Board Secretary for Mease Life (a local CCRC-continuing care retirement community) and board member for the Pinellas County Homeless Leadership Alliance.

Speaker: Lindsey North, MS, RN | NPD-BC Director, Hospital Care Coordination | BayCare Health System

Lindsey North is the Director of Hospital Care Coordination for eight acute care hospitals within the BayCare Health System, a large, non-profit health care system in the greater Tampa Bay area. Lindsey’s departments are responsible for managing hospital length of stay, reducing readmissions, oversight of observation patients, discharge coordination to the appropriate post-acute level of care and contributing to a positive patient experience. Lindsey has worked the past 14 years in healthcare, five of which are in hospital care coordination. In addition to direct patient care, Lindsey also served as a subject matter expert on the EMR implementation across the health system. Lindsey currently serves on three system collaboratives. In 2015, Lindsey authored an article for ANPD and was the designer and instructor for several courses offered across BayCare, including a charge nurse education series, and orientation. She received her Bachelor of Health Science education from University of Florida, her Bachelor of Nursing from University of South Florida and her Master’s in Nursing also from the University of South Florida. Lindsey is an active member of ACMA and is a current Board member for Season’s Hospice in Tampa, Florida.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
