Social Work Navigator Coordinates Care Transitions for Substance Use Population

Abstract
Placement of LCSW Navigators into the acute care setting has been instrumental in working with patients experiencing substance use disorder (SUD) while hospitalized and when connecting patients to treatment in the community. SUD patients have specific needs that, when addressed during the stay, reduce risk of elopement or patients leaving against medical advice, increasing the likelihood of patients connecting with treatment and reducing readmissions. This session will examine challenges and explore options for patients who require long-term IV antibiotics yet have a history of IV drug use. Attendees will gain an understanding of the use of appropriate language to decrease stigmatic culture with SUD patients.

Learning Objectives
1. Explain the Social Work Navigator role within the hospital setting.
2. Identify SUD patient needs and post-acute treatment options to reduce risk of readmission.
3. Explore innovative post-acute placement options for patients with history of IV drug use.
4. Utilize and practice appropriate language to decrease stigmatic culture within the health care setting.

Speaker: Kelli Burba, BSN, RN, ACM-RN | Executive Director, Care Management | AdventHealth

Having worked in the acute care setting for over 30 years, Kelli Burba brings much insight and experience as Executive Director of Care Management for AdventHealth in Central Florida. After starting her career as a bedside nurse, she transitioned to case management where after 13 years she moved into leadership. She currently oversees the strategy and day-to-day operations for a team of over 400 who provide coverage and support for eight hospital campuses and seven free-standing emergency rooms.

Speaker: Michelle Dodson, MA, MSW, C.Ht, LCSW | Strategic Program Manager, Care Management | AdventHealth

Michelle Dodson is a Licensed Clinical Social Worker who works for AdventHealth. She originally began her career in marketing and management but quickly realized her passion for working with the underserved and underprivileged. She attended the University of Central Florida’s master’s in social work. Her experience involves working with HIV+ homeless regarding housing and social issues, children at Nemours Children’s Hospital, adolescents and adults at the University Behavioral Center, and adults in acute care hospital setting at AdventHealth. Her current position is the Strategic Program Manager for the Substance Use Disorder Navigators at AdventHealth, where she leads a team of LCSWs who journey with patients through their time at the hospital and connects them with treatment for their substance use disorder. She has been developing a strategic model for how best to address the needs of patients with substance use for the AdventHealth organization for the past few years.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
