The Next Round-Up: Rethinking the Team

Publication Date: 5/4/2023

Length: 58 minutes 14 seconds

Content Level: 1 Intermediate CE

Abstract
As we move forward toward a post-pandemic state, we can continue to anticipate changes including ongoing initiatives in acute care from a volume-based to a value-based system. The role of case manager as a patient-centric guide for the care team is now more significant than ever. There is a need to find simplicity in the chaos of health care delivery, to rethink the role of the case manager as advocate to coordinate an interprofessional team to best provide the right care, at the right time, in the right setting, at the right cost, with the right outcomes for the patients served. This session will describe how case managers can, with curiosity, perseverance and good old-fashioned grit, acknowledge what we do and do not know. Then apply this to partner with health providers and the patient in a collaborative and coordinated approach to shared decision making around health and social issues.

Learning Objectives
1. Examine pre-pandemic and current mandates of the Affordable Care Act.
2. Identify the impact of COVID-19 on the future of case management practice in value-based care.
3. Explain the concept of the interprofessional team and its impact on the patient and case manager.
4. Apply knowledge gained to rethink the role of the case manager in the formation of an interprofessional team.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
