Transforming Patient Engagement with Motivational Interviewing in a Value-Based System that Demonstrates Measurable Outcomes

Abstract
Value-based practice challenges practitioners to identify ways of improving patient engagement and clinical outcomes. This presentation describes how a financially successful Accountable Care Organization (ACO) integrated motivational interviewing (MI) training for health care professionals into a comprehensive employment orientation for all new primary care coordinators (PCC). Superusers were trained in MI mentorship to support ongoing skill development in clinical practice. A study of MI training methods has identified significant short-term gains in competency with participation in an interactive 12-hour training workshop. Feedback and ongoing supervision helped enhance long-term proficiency. The presentation describes examples of measurable ACO program outcomes, MI training methods and best practices, as well as providing tips for continued learning, including the use of self-evaluation coding instruments, patient surveys and suggestions for independent or small group practice with mentorship support.

Speaker: Robert Scales, Ph.D. | Director of Cardiac Rehabilitation and Wellness, Cardiology | Mayo Clinic

Dr. Robert Scales is a Mayo Clinic Assistant Professor of Medicine and the Director of Cardiac Rehabilitation (CR) and Wellness in the Cardiology Department at Mayo Clinic in Arizona. He is also a volunteer Clinical Professor at Arizona State University. Dr. Scales has a busy practice as a clinical exercise physiologist, and he has counseled thousands of patients recovering from heart disease and related conditions. His primary research interest is the application of MI and effective provider-patient communication in the health care setting. Dr. Scales was the Principal Investigator in the Cardiovascular Health Initiative and Lifestyle Education (CHILE) Study, the first study to investigate the impact of MI on the behaviors of patients attending CR. He was also the Lead Consultant on a National Institutes for Health funded study to investigate methods of teaching MI in an academic physical therapy program. Adaptations of his work using MI with clinical populations have focused on the management of diabetes in Native American communities. He is an experienced MI trainer, and he has trained both students and clinicians from a variety of health care disciplines to use the approach within their own clinical settings. Dr. Scales is a member of the MI Network of Trainers, a Fellow of the American Association of Cardiovascular and Pulmonary Rehabilitation, an advisory board member of the International Concussion Society and a task force member of the Athlete Development and Sports Rehabilitation section of the American Congress of Rehabilitation Medicine.

Speaker: Karen Vanaskie, DNP | Chief Clinical Officer, Care Management | Innovation Care Partners

Karen Vanaskie currently serves as the Chief Clinical Officer for Innovation Care Partners. Her extensive experience in health care leadership, spanning over 30 years, includes clinically integrated health systems, accountable care organizations, inpatient and outpatient settings, home health, multi-specialty medical groups and independent physician associations. She has served in many senior leadership roles designing and directing nursing, case management, quality, outpatient, post-acute and longitudinal care programs. Karen’s formal education includes earning a Baccalaureate Degree in the Science of Nursing (BSN), a Master of Science in Nursing (MSN) and a Doctorate in Nursing Practice (DNP). She is a member of the Case Management Society of America (CMSA) and is a Past President for the Arizona Chapter of ACMA.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
