Patient Choice Transitions of Care and the end of the COVID-19 PHE Waivers

Abstract
This session provides an overview of the history, current state and expected re-enactment of CMS-issued rule CMS-33-17-F and associated Conditions of Participation (CoPs). Included in this discussion is the motivation behind the rule and the expected impact on patient well-being including reduced re-admissions. As we near the end of the Public Health Emergency related to COVID-19, the waivers surrounding this rule will expire and many health systems are already being encouraged to return to pre-PHE practices. This session provides guidance on returning to adherence to the rule.

Learning Objectives
1. Assess CMS Regulations related to transitions to post-acute care including Patient Choice and documentation requirements.
2. Identify ways to develop and enact a plan to support these regulations once the COVID-19 PHE waivers are lifted.
3. List methods for documenting adherence to CMS regulations.
4. Describe how accreditation boards are increasing emphasis on patient engagement in the transitions of care process.
5. Recognize the impact of patient engagement in the transition of care in overall outcomes as well as hospital monitored metrics such as re-admissions, avoidable delays, and length of stay.

Speaker: Marmmett Horton, MSW, ACM-SW, CMAC | Senior Vice President and General Manager | Strata Health

Marmmett Horton holds a Master of Social Work and is a Certified Case Manager through ACMA. She is the Senior Vice President and General Manager of Strata Health. Prior to joining Strata Health, Horton held a series of executive and managerial roles at Dario Health, R1 and Banner Health. She has worked with Case Management leadership at the site and executive level throughout the country in various capacities with efforts geared toward standardization and operational design related to Case Management Department core functions. In her role as a consultant, Horton developed and presented a multitude of educational criteria with a strong focus on regulations, compliance and case management practice. She has also developed curricula focused on change management, process improvement and team building across multiple disciplines. She is an active member of ACMA, holding prior board roles and currently serves on the Fundraising Committee.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
