Leveraging Technology to Reduce Readmissions for Super Utilizers

Abstract
This session provides insight into how one health system leveraged the electronic medical record to create multidisciplinary readmission prevention plans that were triggered upon the patient's presentation to the emergency department. These plans helped the emergency department personnel take appropriate actions to prevent unnecessary admissions. This intervention was successful in driving down readmissions by more than 50% for the organization's top 150 super utilizers.

Learning Objectives
1. Identify parameters for the definition of patients who are considered to be high utilizers of the inpatient environment.
2. Evaluate options in the electronic medical record environment to create a trigger for action when a high utilizer patient presents to the emergency department.
3. Analyze patient circumstances to create a biopsychosocial care plan that can be executed with precision in the emergency department.
4. Review several care plan examples and effective interventions in preventing readmissions for the high utilizer patient population.

Pamela Foster oversees system care management, utilization management, transitional care, home health, home infusion, behavioral health, ethics, and patient transportation for the HonorHealth network in the Scottsdale/Phoenix metropolitan area. She has over 31 years of experience in health care and has been an active member of ACMA and served as National President from 2016-2018. She has a master's degree in social work from Arizona State University and a master's degree in business administration from the University of Phoenix and has held the ACM certification since 2008.

Allison Rose, MHSM, BSN, RN, CPHQ | Senior Network Director, Ambulatory Care Management and Home Health Services | HonorHealth

Allison Rose is a registered nurse and holds a bachelor’s degree in nursing and sociology and a master’s degree in Hospital Administration and Health Sector Management, both from Arizona State University. She oversees disease management programs, transitional care programs, home health services and language and cultural services in the Scottsdale/Phoenix metropolitan area. Prior to this role, she served as the Network Director of Clinical Effectiveness and oversaw multiple projects to support the journey to Fee for Value. Allison has led multiple projects to reduce readmissions for the HonorHealth Network and holds the Certified Professional in Healthcare Quality certification.

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
