Abstract
Elevating health equity among medically complex socially disadvantaged Medicaid populations requires combined knowledge and effort among payers, providers, health care organizations and communities. This session provides examples of innovative programs designed to promote healthy communities among Medicaid health plan members. Keys to success involve hearing the patient voice and building on the collective strengths of each partner to ensure that interventions are responsive to patient/community needs. Aspects of this approach are shared so you can replicate within your community and health care organization.

Learning Objectives
1. Identify needs for underserved populations.
2. Describe successful programs and key success factors.
3. Recognize the value of patient/community perspectives.
4. Promote community, payer and provider engagement to address health disparities for disadvantaged populations.

Karen Vanaskie Scholarship Fund

Karen Vanaskie was one of case management's brightest and most dedicated professionals. She was serving as Secretary / Treasurer on the ACMA National Board of Directors at the time of her unexpected passing on June 21, 2024.

At the request of Karen's family, ACMA has set up the Karen Vanaskie Scholarship Fund. Karen was passionate about the need for case management certification. She actively supported her staff in pursuing the Accredited Case Manager (ACM) Certification. To continue her legacy, this scholarship fund will cover the cost of their ACM certification application fee for those to whom it is awarded. We believe this continues Karen's legacy.
